The evaluation builder turns requirements into a respondent journey and, when enabled, a scoring model. Organise related criteria into sections, use the simplest suitable question type and add logic only when it makes the form clearer. A well-built questionnaire tells respondents what evidence is required and tells reviewers how answers will be interpreted.

Before you start

• Write the evaluation objective, audience and evidence standard before converting a requirements list into questions.

• Choose or create any reusable response sets and decide how not-applicable answers should behave.

Step by step

1. Create meaningful sections

Group questions by topic or review responsibility and order them logically. Use section descriptions for shared instructions rather than repeating the same text on every question.

2. Choose the evidence type

Use choice, scale, rating, text, number, date, ranking, grid or other supported types according to the answer needed. Use file or signature fields only when the workflow and policy require them.

3. Write clear questions

Ask one thing at a time, define the period or scope and explain required evidence. Avoid double negatives and ambiguous scores such as “good” without a definition.

4. Configure responses and scoring

Add choices or a response set, mark legitimate not-applicable options and choose whether the question contributes to scoring. Check score direction before weighting it.

5. Add conditional logic

Show follow-up questions only when the earlier response makes them relevant. Keep dependencies forward-moving and test every branch to avoid hidden required questions.

6. Preview and finish

Use preview as the intended respondent, correct incomplete question warnings and inspect section navigation. Save the draft, but do not send until the form is published.

What happens next

• Configure equal or weighted scoring only after answer meanings are stable.

• Ask a reviewer and a representative respondent to test the preview before the first release.

Troubleshooting

A question is marked unfinished

Complete its prompt and type-specific settings, including required choices, response set or validation, or remove the unused draft question.

A branch creates a dead end

Review every condition and destination, remove circular or conflicting logic and preview each possible trigger answer.

A response option scores in the wrong direction

Open the response set or question scoring, correct the numeric mapping and verify preview results before publishing.
